We aim to help a range of people:Anyone that is thinking of applying for Aged Care services through My Aged Care, or knows of someone who may benefit from support.Anyone who is in the middle of an application but feeling confused, overwhelmed or needs support and advice.Those who have received outcome letters following an assessment and want support with the next steps.Retirement villages, GP clinic staff, Allied Health and Community Service providers who want support and education on how to navigate My Aged Care for their clients.

What Aged Care services can CBR Care Link help with?

We help clients who are eligible for Support at Home, Commonwealth Home Support, GP Chronic Disease Management Plans and private care referrals. We aim to support you to find meaningful services and reliable ongoing care that meets your needs within each of these categories. Find out more here.

Can I be seen in person or virtually?

Accepted Answer

We aim to offer our services in a way that is convenient to you, either in your home or in a mutually agreeable location within the Canberra Region.We also offer virtual consultations* online so that you or your loved ones, who may not be local, can also participate.Although we are based in Canberra, we are more than happy to discuss our supports for clients anywhere within Australia via a virtual platform*.Our usual means of Virtual connection is via Google Meet or MS Teams, but we can also facilitate a videocall to a mobile phone if necessary*.*Please note that not all clinical assessment and support services are advisable to be completed over the phone and we will discuss options with you at the initial free consultation.

Do CBR Care Link offer written documentation?

At CBR Care Link we are dedicated to reducing our ecological impact, and therefore primarily offer our services in an electronic format: via email and e-copies of documentation wherever possible.Printed copies of documentation are available on request, however a small service charge may be applied.

Why does CBR Care Link charge?

Due to the nature of the Support at Home and government funds, we are unable to claim our service fees from any Government means at this stage. Although CBR Care Link is run by clinical staff, we also cannot claim our advisory services through private health insurance at this stage.We aim to keep our fees structured in a transparent flat-fee format and try to ensure that the service remains accessible to all who need it.Our full pricing schedule is available here.
