If you, or a loved one, have received your ACAT decision letter, it can be confusing to understand what's next and how to interpret the information you have been given. CBR Link are here to help.

What is an ACAT Outcome Letter?

An ACAT (Aged Care Assessment Team) outcome letter is the official correspondence sent to you after your comprehensive assessment. It outlines the formal decisions made regarding your eligibility for government-funded aged care services.

Here is a brief summary of what the letter typically contains:

  • Support Plan / Reference Number: It includes your unique My Aged Care client record ID and a summary of your approved service types, which acts as your referral code when speaking with providers.

  • Approval Status: It clearly states whether you have been approved for care, specifying the types of programs you qualify for (such as the Commonwealth Home Support Programme [CHSP], a Support at Home package, or approval for residential aged care/respite).

  • Funding Levels: If approved for a Support at Home package, it will indicate the specific funding tier (e.g., Level 1 through 8) you have been assessed as needing, as well as any additional funding for Restorative Care, Assistive Technology and/or Home Modifications.

  • Next Steps & Validity: It provides referral codes that are included in your approval, and notes any expiration timeframes or review rights if you disagree with the decision.

How CBR Care Link helps:

We support families at every stage of the ACAT process:

  • Before the assessment: Understanding what to expect, what questions to raise, and how to make sure the assessor gets a full picture of your needs

  • During the process: Helping you feel prepared and confident in the room

  • After the assessment: Explaining the outcome in plain language and identifying the right next steps — whether that's applying for a Support at Home Package, exploring residential care, or understanding your options further.

You will be supported by a Registered Nurse with extensive experience in aged and community care. We know the ACAT process from the clinical side and from the family side. Our job is to make sure you're not navigating it alone.
